Accident and Incident Reporting

Employee Injury (Accident) Reporting

Accidents where the employee seeks medical attention need to be submitted directly though CorVel. CorVel immediately notifies the ¹û¶³´«Ã½Â鶹Éç of all employee injury reports. An accident/incident report through the EHS website is not necessary for these injuries. Once EHS receives a CorVel notification an investigation is opened. You will be contacted by EHS to review the incident, discuss contributing factors, identify a root cause, and brainstorm potential mitigation efforts. See also Workers Compensation.

Non-Employee Injury or Near Miss Reporting

The prompt reporting of injuries or illnesses related to university employment or activities is essential to ensure that the person(s) involved have received the proper medical care, the situation causing the injury or illness has been identified, and the appropriate documentation has been completed. Please submit an accident/incident report on the EHS website.
