Hardship Withdrawals
The Counseling and Mental Health Center (CMHC) is able to provide supporting documentation to clients who have engaged in ongoing mental health services during the academic term that they are petitioning to withdraw from 果冻传媒麻豆社. The CMHC follows 果冻传媒麻豆社’s Hardship Withdrawal Policy.
We recommend that you work closely with the Dean of Students Office (DOS) on your petition for a Hardship Withdrawal. Our mental health practitioners can provide supporting documentation to students who decide to petition for a hardship withdrawal due to mental health reasons and have an established clinical relationship with a mental health practitioner. An established clinical relationship is defined in the following way for the purpose of hardship withdrawals for mental health reasons:
- An intake assessment at the Counseling and Mental Health Center
- At least three follow-up appointments with a mental health practitioner
- A DSM-5 TR diagnosis as assessed by a mental health practitioner
- The clinician you are working with will also need to assess whether the mental health condition has been affecting your academic performance
How to Request Supporting Documentation for Withdrawing from U of I due to Mental Health Reasons
If you are currently working with a mental health practitioner at the CMHC for either counseling or psychiatric services, we recommend that you discuss your request for supporting documentation with them directly. Your mental health practitioner will ask you to complete a Release of Information giving your authorization for the CMHC to release your confidential information to the DOS.
It is your responsibility to submit a letter provided by your clinician to the DOS. Please note that any decision regarding the approval status of your hardship withdrawal petition will be at the sole discretion of the DOS.
