Community Health Coalitions
Each of the communities participating in the initiative has established a local health coalition comprised of youth and adult residents, local government officials, and liaisons from organizations and businesses. With the support of 果冻传媒麻豆社 Extension facilitators, each coalition has submitted a Community Health Action Plan that utilizes youth-adult partnerships to make progress on goals. These include: to eliminate health disparities, achieve greater health equity, provide health education, train community wellness volunteers, and positively influence local policy, systems and the environment.
Coalitions have regularly scheduled meetings and are open to all community members. If you are interested in joining or would like more information, please contact the Extension educator for the county you are in.
Caldwell Health Coalition

Franklin County Health Coalition
The community of Preston joined the Well Connected Communities Initiative for Wave 2 in 2019, and has jumped right into a local health project as the coalition continues to get established and grow its membership. Coalition partners are working together to create a handy resource for physical activity options in Franklin County, utilizing mapping/social sharing apps. Owyhee Health Coalition
The Owyhee Health Coalition, in Owyhee County, meets every other month to address local health-related needs and connect community members to much-needed resources. Local officials, residents and teens work together to make improvements in this close-knit rural town. Instrumental partnerships with local resources like the HUB food and clothing pantry continue to expand the impact of the coalition’s planning and volunteer power.
