Select Agent and Toxin Concerns
To provide a mechanism for you to report safety, security or other concerns associated with Select Agents and Toxins, the Department of Health & Human Services, Office of Inspector General (OIG) maintains a hotline which offers a confidential means for reporting this vital information. When reporting these issues to OIG, please ensure that you indicate it is a "Select Agent Program" issue.

Reporting Concerns
Unethical, illegal or unsafe acts can have serious consequences for the university and its employees, including financial losses, legal liability, reduced morale and diminished public trust. Each employee shares the responsibility for preventing these effects by averting the causes.
Note: Allegations of noncompliance will remain confidential, to the extent permitted by law and consistent with the need to conduct an adequate investigation of the allegations. The university will take measures to protect from adverse actions or retaliation against any person who, in good faith, makes allegations of noncompliance under this policy. (See FSH 3290 and 3810.)
